Convulsions (including epilepsy) in childhood.

چکیده
Of all the upsets which can happen to the infant or child, it would be safe to say that none is as alarming to the parents as a convulsion. A good knowledge of the causes of convulsions is therefore essential, if one is to handle the situation well, and relieve the parental anxiety with authority. The diagnosis may sometimes be difficult, for even 'windy pains ' with screaming may simulate a convulsion in the parental mind. More often than not, we must base our diagnosis on the description given to us by the parents, as the attack is over before we see the child. Petit mal may be so fleeting that it can, for some time, be mistaken for a mannerism by the parents. When called to a child with a convulsion nothing can replace a very careful physical examination. The throat and ears should be viewed to exclude infection; the heart, lungs, and abdomen should also be examined, and, when possible, a specimen of urine should be analysed for sugar, acetone, albumen, blood, or pus. The eye grounds should be scrutinized for papilloedema and it should be ascertained whether there is a squint; the remainder of the central nervous system should be run over, and this examination should include testing for Kernig's sign. An examination of the nervous system of an infant or child is less informative than that of an adult, as the presence or absence of, for example, the knee jerks, is not so significant. I propose describing what is usually called epilepsy ' last of all. Since convulsions are a symptom and not a disease, it would be well to begin by enumerating the various conditions which will give rise to this symptom.

Febrile Convulsions in Children: Demographic Features, Causes and Seasonal Pattern
Background and Objectives: Febrile convulsion (FC) is the most common seizure disorder in childhood period. Some studies have suggested association between seasons and occurrence of FC. This study was designed to determine demographic features, causes and seasonal variations in cases with FC. Method: In this descriptive, cross sectional study, 282 children 6- 60 months with FC were admitted to ...
متن کاملA splice-site mutation in GABRG2 associated with childhood absence epilepsy and febrile convulsions.
CONTEXT Missense mutations in the GABRG2 gene, which encodes the gamma 2 subunit of central nervous gamma-aminobutyric acid (GABA)(A) receptors, have recently been described in 2 families with idiopathic epilepsy. In one of these families, the affected individuals predominantly exhibited childhood absence epilepsy and febrile convulsions. OBJECTIVE To assess the role of GABRG2 in the genetic ...
متن کاملLong term outcome of prophylaxis for febrile convulsions.
A cohort of 289 children with febrile convulsions who had been randomised in early childhood to either intermittent prophylaxis (diazepam at fever) or no prophylaxis (diazepam at seizures) was followed up 12 years later. The study focused on the occurrence of epilepsy and on neurological, motor, intellectual, cognitive, and scholastic achievements in the cohort. At follow up the two groups were...
متن کاملSpontaneous fits after convulsions with fever.
112 of an original sample of 134 children with febrile convulsions were reviewed between 8 years and 9 years 10 months after their initial attack. 17% of those followed up had had at least one spontaneous fit. A significant correlation was found with perinatal abnormalities. 12% had continuing recurrent fits. Persisting grand mal occurred most commonly in lower social class children who had had...
متن کاملIdiopathic familial temporal lobe epilepsy with febrile convulsions
Efforts to duplicate the genetic and molecular breakthroughs of autosomal dominant frontal lobe epilepsy have lead to increased interest in familial temporal lobe epilepsy. In this report we describe three kindreds. The epilepsy syndrome described manifests after the teenage years and was generally mild and treatment responsive. The predominant seizure types were simple and complex partial seiz...
متن کاملBasic Mechanisms Underlying Seizure-prone & Seizure Resistant Sleep and Awakening States in Feline Kindled and Penicillin Epilepsy
Epilepsy is a chronic neurological disorder which is manifested at some times and masked at others. Sleep-waking state physiology is one of the most well documented factors affecting the clinical expression or suppression of human epilepsy . Specifically, non-rapid-eye-movement (NREM) sleep and the gradual process of awakening from NREM sleep are the most vulnerable periods for seizures, especi...
